Job Title: Community Treatment Team Practitioner

We are seeking a highly skilled and experienced Community Treatment Team Practitioner to join our team at NELFT NHS Foundation Trust. As a Community Treatment Team Practitioner, you will be responsible for assessing and managing patients with complex needs in the community, working closely with other healthcare professionals to provide high-quality care.

Main Responsibilities:

  • Assess and manage patients with complex needs in the community
  • Work closely with other healthcare professionals to provide high-quality care
  • Develop and implement individualized care plans to meet the needs of patients
  • Provide education and support to patients and their families to promote self-management and independence
  • Collaborate with other healthcare teams to ensure seamless care transitions and coordination

Requirements:

  • Registered Nurse or Paramedic with a minimum of 1 year of experience in a band 6 post or equivalent
  • Portfolio evidence of professional development and commitment to CPD
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Commitment to ongoing training and education
