Community Treatment Team Practitioner

About the Role:
We are seeking a highly skilled and experienced Community Treatment Team Practitioner to join our team at NELFT NHS Foundation Trust. As a key member of our Community Treatment Team, you will play a vital role in providing high-quality care to patients in their own homes, reducing the need for hospital admissions and improving patient outcomes.
Key Responsibilities:
- Assess and manage patients with complex needs, providing comprehensive care and support to ensure the best possible outcomes.
- Work collaboratively with other healthcare professionals, including GPs, hospital staff, and social services, to provide seamless care and support to patients.
- Develop and implement individualized care plans, taking into account the unique needs and circumstances of each patient.
- Provide education and support to patients and their families, empowering them to take an active role in their care and recovery.
- Contribute to the development and implementation of new care pathways and services, ensuring that they are evidence-based and meet the needs of our patients.
Requirements:
- Registered Nurse or Paramedic with a minimum of 1 year's experience in a similar role.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Strong assessment and problem-solving skills, with the ability to think critically and make sound judgments.
- Ability to work independently and as part of a team, with a flexible approach to working hours and locations.
- Knowledge of current evidence-based practice and guidelines, with a commitment to ongoing professional development and education.
