Reconnect Navigator: Pathway to Recovery

At Humankind, we are committed to addressing health inequalities in England by providing free, confidential support for individuals struggling with alcohol, drugs, housing, or mental health issues. As a Reconnect Navigator, you will play a vital role in promoting continuity of care and supporting service users in their recovery journey.
The Specialist Healthcare Navigation (Enhanced Reconnect) service aims to reduce health-related barriers that hinder engagement and promote positive connections within local communities. You will work closely with medium to high-risk offenders, encouraging them to link with partners and services, and access various opportunities on release.
This rewarding role involves managing an adult caseload, delivering person-centered care, and collaborating with key partners to achieve the best health and wellbeing outcomes for individuals. Key duties include:
- Co-producing and reviewing recovery and treatment plans
- Facilitating progression from initial engagement to self-sustained recovery
- Reducing alcohol/drug-related harm to individuals and the wider community
The ideal candidate will have excellent communication skills, be able to work effectively in a team, and possess a strong understanding of the complexities surrounding substance misuse and recovery. Please note that this position requires regular evening work and occasional weekend work, with a rota-based schedule. A full driving license and the ability to travel across the North East region are essential.
We offer a competitive salary of £35,000 - £40,000 per annum, depending on experience, as well as a range of benefits, including generous annual leave entitlement, flexible working options, and a supportive working environment.
