Reconnect Navigator Role

At Humankind, we offer a unique opportunity to work with individuals in need of drug and alcohol recovery services. Our team provides holistic, collaborative care to men and women in custody across the North East region.
The Healthcare Navigation Reconnect Service aims to reduce health-related barriers that impact individuals' engagement prior to and upon leaving prison. As a Reconnect Navigator, you will promote continuity of care support for those leaving prison, encouraging positive and sustainable connections within their local communities and building recovery capital assets.
Key responsibilities include:
- Promoting continuity of care support for individuals leaving prison
- Encouraging positive and sustainable connections within local communities
- Building recovery capital assets
- Providing peer support and making personal support plans
- Assisting with healthcare access and navigation
