Job Title: Reconnect Navigator

Humankind is seeking a skilled and compassionate Reconnect Navigator to join our team. As a key member of our Reconnected to Health service, you will play a vital role in supporting individuals in custody to achieve their recovery goals and reintegrate into their communities.

About the Role

The Reconnect Navigator will work closely with our clinical partners and colleagues to provide holistic, collaborative care to men and women in custody across the North East region. Your primary responsibility will be to promote continuity of care support for those leaving prison, encouraging positive and sustainable connections for service users within their local communities and building recovery capital assets.

Key Responsibilities
  • Manage an adult caseload of service users, delivering person-centred care.
  • Co-produce and review recovery and treatment plans.
  • Facilitate and promote individuals' progression from initial engagement, through structured treatment, to self-sustained recovery.
  • Reduce alcohol/drug-related harm to individuals and the wider community.
  • Work with service users towards detoxes both community and in-patient as well as rehabilitation centres.
  • Collaborate closely with key partners and colleagues to achieve the best health and wellbeing outcomes for individuals.
Requirements

To be successful in this role, you will need:

  • A strong understanding of substance misuse and mental health issues.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced, dynamic environment.
  • Full driving licence and access to a motor vehicle.
