About the Role

We are seeking a skilled Healthcare Navigation Specialist to join our team at Humankind. As an integral part of our Reconnected to Health service, you will play a vital role in promoting continuity of care support for individuals leaving prison, encouraging positive and sustainable connections within their local communities.

Job Description

In this challenging yet rewarding position, you will be responsible for:

  • Navigating healthcare services for individuals in custody, ensuring seamless transitions upon release.
  • Building recovery capital assets through peer support, personal support plans, and access to community resources.
  • Collaborating with healthcare staff in the community to provide comprehensive support networks.
  • Facilitating transportation and appointment arrangements to ensure timely access to healthcare services.
Required Skills and Qualifications

To succeed in this role, you will need:

  • A strong understanding of the healthcare system and community resources.
  • Excellent communication and interpersonal skills.
  • The ability to work effectively in a fast-paced environment.
  • A full driving license and access to a motor vehicle.
