This role involves working closely with our team to provide clinical leadership, advice, support, and information to staff on all aspects of heart failure care. The post holder will be responsible for facilitating the early identification of patients admitted to hospital with suspected or confirmed heart failure, as well as admission prevention management. This includes supporting community patients, running outpatient clinics, and conducting relevant home visits as part of a team approach to care delivery and caseload management.

The successful candidate will have a strong background in cardiology and experience in managing Heart Failure conditions. They will also have an in-depth knowledge of the clinical governance agenda, including clinical risk and safety. Working towards independent prescribing qualification is highly desirable.

In addition to your clinical expertise, you will have excellent communication and interpersonal skills, with the ability to work effectively with colleagues at all levels. You will also be able to utilize advanced nursing skills and have the knowledge to identify cardiac disease.

We offer a competitive salary of £35,000 - £45,000 per annum, depending on experience, plus a range of benefits including generous annual leave entitlement, pension scheme, and access to NHS discount schemes.
