Job Title: Social Support Professional

About the Role:

We are seeking an experienced and empathetic Social Support Professional to join our team at Thrive Together Social Care. As a key member of our multidisciplinary team, you will work closely with clients and their families to provide tailored support and guidance.

Key Responsibilities:

  • Conduct assessments to identify client needs and develop comprehensive support plans.
  • Provide emotional support and guidance to individuals and families facing various challenges.
  • Facilitate access to community resources, services, and benefits for clients.
  • Maintain accurate records of client interactions and progress in accordance with organisational policies.
  • Collaborate with other professionals to ensure comprehensive support for clients.
  • Supervise and mentor junior staff members to promote professional development.
  • Stay informed about relevant policies, legislation, and best practices in social care.

Requirements:

  • A recognised qualification in social work, such as a degree or diploma, and registration with Social Work England.
  • 3 years post-qualified experience.
  • Successful completion of the ASYE programme.
  • A full driving licence and access to a vehicle for work purposes.
  • Strong interpersonal skills and ability to build rapport with clients from diverse backgrounds.
  • Excellent communication skills, both verbal and written.
  • Ability to work independently and as part of a multidisciplinary team.
