We are seeking compassionate and dedicated professionals to join our team at Thrive Together Social Care. As a key member of our multidisciplinary team, you will play a vital role in supporting individuals and their families in navigating social challenges.

About the Role
This position involves working closely with clients to assess their needs, provide guidance, and connect them with appropriate resources and activities. You will also be responsible for supervising and mentoring junior staff members, ensuring a high standard of care and support is maintained.

Responsibilities
  • Conduct thorough assessments to identify clients' needs and develop tailored support plans.
  • Provide counselling and emotional support to individuals and families facing various challenges.
  • Facilitate access to community resources, services, and benefits for clients.
  • Maintain accurate records of client interactions and progress in accordance with organisational policies.
  • Collaborate with other professionals to ensure comprehensive support for clients.
  • Supervise and mentor junior social work staff, providing guidance on best practices and professional development.
  • Participate in case reviews and team meetings to discuss client progress and strategies for improvement.

Requirements
  • A recognised Social Work qualification (e.g., Degree in Social Work, PGC in Social Work) and registration with Social Work England.
  • 3 years post-qualified experience.
  • ASYE completion.
  • A full driving licence and access to a vehicle for work purposes.
  • Strong interpersonal skills, excellent communication skills, and ability to work independently and as part of a team.
