About the Role

This Care and Community Liaison position offers the opportunity to make a meaningful impact on individuals' lives while developing valuable skills in the field of family support work.

The successful candidate will be responsible for promoting independence and engagement in new activities for children and young people with disabilities, as well as providing support and guidance to individuals in need.

Estimated Salary: £21,600 - £29,400 per annum (based on 12-18 hours per week)

Duties and Responsibilities
  1. Develop and implement care plans in collaboration with other team members.
  2. Provide respite for families by offering support and supervision during outings and activities.
  3. Conduct assessments to identify needs and risks.
  4. Collaborate with other professionals to ensure comprehensive care.
Requirements and Qualifications
  • NVQ Level 3 or above is preferred.
  • A valid driving license is required.
  • Previous experience working with children or individuals with disabilities is desirable.
