Join our team as a Community Care Practitioner and play a vital role in supporting individuals with complex mental health needs in Cheshire West. As an Occupational Therapist Specialist, you will work intensively with patients, providing specialist occupational therapy assessment and intervention to help them regain skills and confidence to live successfully in the community.

You will work closely with a multidisciplinary team to ensure effective patient care, with a strong focus on recovery, goal setting, and promoting independence. This role requires excellent communication and risk management skills, as well as the ability to work under pressure and manage own caseload.

With a salary of £30,000 - £40,000 per annum, depending on experience, this is an excellent opportunity to make a real difference in the lives of individuals with complex mental health needs.

The ideal candidate will have a Diploma or BSc in Occupational Therapy, registration with the Health Professions Council, and accreditation as a Practice Placement Educator. You will also need at least 18 months' experience in mental health, with excellent verbal and written communication skills.
