About the Job:
We are seeking a highly motivated and compassionate Person-Centred Care Practitioner to join our team in Chester. As a key member of our team, you will be responsible for providing high-quality care and support to individuals with autism and learning disabilities.
The ideal candidate will have a passion for delivering person-centred care and possess excellent communication skills. Knowledge of positive behaviour support, LD, and autism would be beneficial, but all training will be provided.
Key responsibilities include:
- Supporting individuals with daily living tasks
- Encouraging independence and autonomy
- Developing and implementing individualised care plans
Salary: £27,200 - £33,960 per annum (dependent on experience), plus benefits.
