Position Overview

We are seeking a highly skilled and autonomous Senior Nurse Practitioner to join our dedicated team at Primary Care Management Solutions Ltd. This role involves delivering comprehensive nursing services as part of a collaborative multidisciplinary team, ensuring high-quality care for our patient population within the defined scope of practice.

Key Responsibilities

The Senior Nurse Practitioner will undertake a variety of clinical responsibilities, which may include but are not limited to:

  • Health promotion initiatives
  • Management of chronic health conditions
  • Preventative health strategies
  • Conducting well-woman and well-man clinics
  • Collaborating with the management team to review and enhance clinical policies and procedures

Core Duties

The following are the essential duties expected of the Senior Nurse Practitioner. Additional tasks may be assigned based on operational needs:

  • Provide expert clinical care to patients
  • Engage in health education and promotion activities
  • Conduct assessments and develop care plans
  • Monitor and evaluate patient outcomes
  • Maintain accurate and comprehensive patient records
  • Collaborate with other healthcare professionals to ensure integrated care delivery

Qualifications and Experience

Required Qualifications

  • Registered Nurse with a valid license
  • Advanced Practice Qualification (postgraduate diploma or degree)
  • Certified Nurse Prescriber
  • Qualified in Triage Nursing

Preferred Qualifications

  • Minor Illness Certification
  • Teaching Certification
  • Advanced Life Support (ALS) and Pediatric Advanced Life Support (PALS) certifications

Experience Requirements

Essential Experience

  • Proven experience in a primary care setting
  • Expertise in chronic disease management
  • Proficient in prescribing and conducting medication reviews

Desirable Experience

  • Background in practice nursing or community nursing

Clinical Competencies

  • Proficient in wound care and suture/staple removal
  • Skilled in performing ECGs
  • Competent in venepuncture
  • Capable of conducting new patient assessments
  • Knowledgeable in chaperone procedures
  • Experienced in requesting and interpreting pathology tests
  • Familiar with travel medicine protocols
  • Knowledgeable in managing diabetes, hypertension, and asthma
  • Proficient in spirometry and cardiovascular health
  • Experienced in immunization practices
  • Understanding of women's health issues
  • Committed to evidence-based practice
  • Knowledgeable in clinical governance
  • Ability to document accurate clinical notes
  • Capable of working within defined practice boundaries and recognizing when to refer to physicians
  • Understanding of health promotion strategies
  • Familiar with Patient Group Directions (PGDs) and related policies
