Job Summary
We are seeking an experienced Advanced Nurse Practitioner to join our team at Primary Care Management Solutions Ltd. As an autonomous practitioner, you will be responsible for delivering high-quality practice nursing services, working collaboratively with our multidisciplinary team to provide care to our patient population.
Main Responsibilities
The successful candidate will be responsible for a number of clinical areas, including health promotion, chronic disease management, health prevention, and well-women and well-man clinics. You will also support the management team in reviewing clinical policy and procedure.

- Manage chronic disease and provide health prevention services
Person Specification
We are looking for an experienced Advanced Nurse Practitioner with a strong background in primary care. The ideal candidate will have a postgraduate diploma or degree in Advanced Practice, as well as experience in chronic disease management and prescribing. They will also have a broad knowledge of clinical governance and be able to work within their own scope of practice.
