Advanced Nurse Practitioner

We are seeking an experienced Advanced Nurse Practitioner to join our team at Primary Care Management Solutions Ltd. As an Advanced Nurse Practitioner, you will be responsible for delivering high-quality patient care and working collaboratively with our multidisciplinary team to provide comprehensive healthcare services.
Main Responsibilities- Deliver practice nursing services as an autonomous practitioner, working within your scope of practice and referring to GPs when necessary.
- Manage chronic disease, health promotion, and prevention services, including well-women and well-man clinics.
- Support the management team in reviewing clinical policy and procedure.
- Participate in audit and quality improvement initiatives as directed by the audit lead.

Person Specification- Registered Nurse with a postgraduate diploma or degree in Advanced Practice.
- Qualified Nurse Prescriber and Triage Nurse.
- Experience working in primary care, chronic disease management, and prescribing.
- Knowledge of health promotion strategies, clinical governance, and evidence-based practice.
