Job Summary
We are seeking an experienced Advanced Nurse Practitioner to join our team at Primary Care Management Solutions Ltd. As an autonomous practitioner, you will be responsible for delivering high-quality practice nursing services, working collaboratively with our multidisciplinary team to provide care to our patients.
Main Responsibilities
The successful candidate will be responsible for:
- Delivering practice nursing services, including health promotion, chronic disease management, and health prevention
- Working as part of the practice multidisciplinary team to provide care to patients
- Supporting the management team in reviewing clinical policy and procedure
- Participating in audit and quality improvement initiatives

Person Specification
The ideal candidate will have:
- Registered Nurse qualification
- Postgraduate diploma or degree in Advanced Practice
- Qualified Nurse Prescriber status
- Experience working in a primary care environment
- Experience in chronic disease management and prescribing
