Social Services Practitioner

Care practitioners play a vital role in ensuring that individuals with complex needs receive the support they require to thrive. We are looking for a motivated and caring individual to join our team at Cornerstone Community Care.
In this position, you will provide day-to-day support to individuals living in their own flats. This will involve helping them manage their daily routines, encouraging independence, and promoting social and health wellbeing.
A successful candidate will have an understanding of professional boundaries, accountabilities, and responsibilities. They should also be passionate about delivering excellent person-led care and support.
Shift patterns typically fall between 0800-2200 hours, with occasional variations. Weekends, public holidays, and waking nights may also form part of your rota. However, we prioritize work-life balance and offer flexibility where possible.
Salary: £26,500 - £32,000 per annum depending on experience
