About the Role:
Chester Outreach Support service is seeking a dedicated and compassionate individual to join our team as a Community Support Worker. As a key member of our team, you will provide support to individuals living in the community who are experiencing mental health issues.
Key Responsibilities:
- Provide support to individuals in line with agreed social care assessments and individualised support and risk plans.
- Enable individuals to integrate into the community and form meaningful relationships.
- Work alongside social workers and community mental health teams to reduce mental health issues.
- Provide support with activities, managing tenancies, finances, bill payments, medication management, shopping, and household tasks.
