About the Opportunity

The Lancashire & South Cumbria NHS Foundation Trust is seeking a highly motivated and experienced Mental Health Practitioner to join their team. This exciting opportunity offers the chance to work with vulnerable service users, providing care and treatment in their own homes.

As a Mental Health Practitioner, you will be part of a close-knit team that works closely with Community Mental Health Teams and other services across the network. Your expertise will enable you to work sensitively and effectively with service users who are experiencing complex mental health needs.

This role requires a strong commitment to delivering high-quality care, ensuring that service users receive the support they need to manage their conditions and improve their overall wellbeing.

Main Responsibilities
  • To provide holistic assessment and care management skills for individuals with complex mental health needs
  • To develop and implement effective interventions and treatment plans tailored to individual needs
  • To collaborate with multidisciplinary teams to ensure seamless delivery of care
  • To maintain accurate records and adhere to best practice guidelines
Requirements

We are seeking candidates with:

  • Registration as a Registered Mental Health Nurse, Social Worker or Occupational Therapist
  • Ongoing professional registration
  • Knowledge and experience of care pathways for psychosis and non-psychosis, and relevant interventions
  • Excellent communication and interpersonal skills
