About the Role

We are seeking a highly skilled and dedicated Community Health Nurse to join our Out of Hours Team at Cambridgeshire and Peterborough NHS Foundation Trust.

Key Responsibilities
  • Provide high-quality nursing care and interventions to patients in their own homes, working autonomously as part of a multidisciplinary team.
  • Assess, plan, implement, and evaluate treatment to patients within professional guidelines, prioritizing patient-centered care.
  • Collaborate with colleagues and other agencies to ensure seamless care delivery and effective communication.
  • Contribute to risk assessments and health and safety assessments, reporting any changes or newly identified risks.
  • Maintain accurate and timely written records, adhering to Trust policies and NMC guidelines.
  • Participate in multidisciplinary team meetings and case reviews to ensure comprehensive care planning.
Requirements
  • RGN level 1 and RGN level 2 qualifications.
  • Evidence of professional development and commitment to ongoing learning.
  • Experience in an acute or primary care setting, with newly qualified staff considered with relevant placement experience.
  • Ability to work flexibly as part of a team, with a strong focus on patient-centered care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and colleagues.
