Community Nurse Specialist

About the Role
We are seeking a highly skilled and dedicated Registered Nurse to join our Out of Hours Community Nursing Team in Cambridgeshire. As a Community Staff Nurse, you will be working as an autonomous practitioner within the out of hours countywide team, providing high-quality nursing care and interventions to patients in their own homes.
Key Responsibilities
- To be professionally and legally accountable for all aspects of your own and delegated work.
- To work within codes of practice and professional guidelines.
- To use evidence-based practice to ensure clinical care is effective and appropriate.
- To promote independence and reduce hospital admissions by providing care in the most appropriate setting.
- To contribute to risk assessments and health and safety assessments, and report any changes or newly identified risks.
- To effectively communicate with patients, carers, and colleagues using tact and persuasive skills.
- To work effectively within the Out of Hours Team structure and liaise with members of the MDT and other agencies.
- To maintain accurate and timely written records, and report any issues as appropriate.
