Job Summary

We are seeking a highly skilled and dedicated Community Staff Nurse to join our Out of Hours Countywide Team in Cambridgeshire. As a Community Staff Nurse, you will be working as an autonomous practitioner, providing high-quality nursing care and interventions to patients in their own homes.

About the Role

This is an exciting opportunity to work in a dynamic and supportive team, providing holistic care to patients across the county. You will be responsible for assessing, planning, implementing, and evaluating treatment to patients within professional guidelines. You will also have the opportunity to provide support and education to peers, new staff, and students.

Key Responsibilities
  • To be professionally and legally accountable for all aspects of your own and delegated work.
  • To work within codes of practice and professional guidelines.
  • To use evidence-based practice to ensure clinical care is effective and appropriate.
  • To promote independence and reduce hospital admission and inpatient length of stay.
  • To contribute to risk assessments and health and safety assessments.
  • To effectively communicate verbally with patients, carers, and colleagues.
  • To work effectively within the Out of Hours Team structure and liaise with members of the MDT and other agencies.
  • To ensure accurate and timely written records are kept.
Requirements

To be successful in this role, you will need:

  • RGN level 1 and RGN level 2 qualifications.
  • Evidence of professional development.
  • Experience in an acute or primary care setting.
  • Ability to work flexibly as part of a team.
  • Ability to assess, plan, and implement nursing care.
  • Ability to teach and support student nurses and HCA's.
  • Ability to safeguard and promote the welfare of children and young people/vulnerable adults.
