", "Key Responsibilities": "As a Community Staff Nurse, you will be working as an autonomous practitioner within the out of hours countywide team.
You will provide high standard of nursing care and interventions within the patient's own home, with support from team leaders.
The patient will be at the centre of the care being provided, ensuring patient and carer participation in decision making.
You will be accountable for assessing, interpreting, planning, implementing, and evaluating treatment to patients within professional guidelines. Patient needs will be identified through holistic nursing.
