Community Staff Nurse

We are seeking a highly skilled and dedicated Registered Nurse to join our Out of Hours Community Nursing Team in Cambridgeshire. As a Community Staff Nurse, you will be working as an autonomous practitioner within the countywide team, providing high-quality nursing care and interventions to patients in their own homes.
Key Responsibilities- Assess, plan, implement, and evaluate treatment to patients within professional guidelines.
- Provide high-standard nursing care and interventions within the patient's own home, with support from team leaders.
- Work effectively within the Out of Hours Team structure and liaise appropriately with members of the Multi-Disciplinary Team (MDT) and other agencies.
- Ensure accurate and timely written records are kept, complying with Trust policy and NMC guidance.
- Inform/update all members of the MDT, service users, and appropriate others, of changes involving current care plans, patient progress, and other relevant matters.
- RGN level 1 and RGN level 2 qualifications.
- Evidence of professional development.
- Experience in an acute or primary care setting, although newly qualified staff may be considered with relevant placement experience.
- Ability to work flexibly as part of a team.
- Able to assess, plan, and implement nursing care.
- Teach and support student nurses and HCA's.
- Accepts responsibility and accountability for own work and can define the responsibilities of others.
