Job Overview

We are seeking a highly skilled Nursing Associate to join our Out of Hours Countywide community nursing team. As a key member of the multi-disciplinary team, you will provide high-quality nursing care and interventions to patients with clinical health needs in their own homes.

Working closely with the neighbourhood team, you will be responsible for assessing patients, formulating care plans, and delivering evidence-based interventions to improve patient outcomes.

The ideal candidate will have experience in community nursing, palliative care, and care of patients with diabetes. You will also possess excellent communication and interpersonal skills, with the ability to work flexibly and as part of a team.

Main Duties of the Job

Please note that this role requires independent travel around the county, meeting strict time deadlines. You will need to hold a full UK driving licence and have access to a vehicle.

We are looking for candidates who have:

  • Experience of community nursing, palliative care, and care of patients with diabetes
  • Education to Registered Nursing Associate level on the NMC Register
  • Experience in falls prevention work or willingness to gain skill
  • Experience of caring for and supporting adults and older people
  • Ability to demonstrate a commitment to high-quality service provision
  • Ability to work flexibly and as part of a team, as well as individually
  • Ability to travel independently using own vehicle
  • Good time management and ability to manage individual workload
  • Enhanced communication and interpersonal skills
