Job Summary

We are seeking a highly skilled and experienced Specialist Band 6 Multiple Sclerosis Community Nurse to join our Huntingdonshire Multiple Sclerosis Service. As a key member of our team, you will be responsible for delivering high-quality, evidence-based care to patients with Multiple Sclerosis, working closely with the clinical lead and multidisciplinary team.

Main Responsibilities
  • Manage a clinical caseload across Huntingdonshire, providing holistic nursing provision to patients with Multiple Sclerosis.
  • Work collaboratively with the wider multidisciplinary team, including the Integrated Early Supported Discharge for Stroke and Community Neuro Rehab Team, and the wider CPFT services.
  • Deliver specialist assessments, Annual Reviews, and Holistic Reviews, reducing the need for hospital admission under the supervision of the Lead MS Nurse Specialist.
  • Communicate and collaborate with multidisciplinary teams to ensure a coordinated service, representing the MS team at meetings, case conferences, and discharge planning.
  • Follow service users and their carers from diagnosis through palliative care, working across health and social care pathways.

Requirements
  • Current NMC Registration.
  • Documented evidence of own continuing professional development.
  • Two years post-registration experience.
  • A basic theoretical and practical knowledge of MS.
  • Experience of working in a multi-disciplinary team.
