As a key member of our community services team, you will be responsible for receiving and processing referrals for our services, ensuring timely and efficient delivery of care to our patients.
Main Responsibilities- Receive and process referrals for community services, collecting relevant information and completing contact assessments.
- Accurately record referrals on RIO and Adastra clinical platforms.
- Ensure referrals are processed in a timely manner and signposted to the appropriate service.
- Provide excellent customer service, answering phone calls and responding to inquiries.
- Work collaboratively with the Triage Team and administrators to manage referrals and ensure seamless delivery of care.
- NVQ level 3 qualification or equivalent.
- Excellent computer skills, including Microsoft products and ECDL.
- Working knowledge of Excel, Word, and Outlook at an intermediate or advanced level.
- GCSE English and Maths grade C and above.
- Ability to work under pressure, prioritize tasks, and maintain confidentiality.
