As a Community Support Specialist in Wokingham, you will play a vital role in providing support to clients who are unable to access our service. This part-time contract is an excellent opportunity for individuals with a passion for helping others.

About the Role

The role requires working 21-28 hours per week, visiting clients at their homes or in hospital settings to provide ongoing support and assessments. You will work closely with partner agencies and other professionals to ensure comprehensive care for our clients.

Responsibilities
  • Conduct home visits for clients with limited mobility to complete assessments and keywork.
  • Visit clients who are off script/not in treatment to attempt re-engagement.
  • Provide support to clients in hospital settings as needed.
  • Assist with on-site assessments, duty, and clinic cover when required.
  • Pursue joint work with partner agencies as necessary.
About You

We are seeking an experienced individual with a proven track record of providing high-quality services to individuals with complex needs. Your enthusiasm for working with disadvantaged or socially excluded people is essential, along with strong IT skills and the ability to use digital meeting platforms, Microsoft Office applications, and case management systems.

Requirements
  • Ability to work one-on-one or lead group sessions, complete assessments, care and support plans, risk assessments, and case recording in a timely manner and to a high degree.
  • Strong communication and interpersonal skills, with the capacity to remain composed under pressure.
  • Able to meet deadlines and properly record information, write reports, and engage with colleagues effectively.
Salary and Benefits
