Job Summary

We are seeking an experienced and passionate Service Delivery Coordinator to support our services in Totnes.

This is a part-time, 25 hours per week, permanent position that offers a competitive salary of £15.77 per hour and a welcome bonus of £250 for new starters in Devon.

As a Service Delivery Coordinator, you will work with a dedicated team, supporting adults with learning disabilities and autism in their daily lives. Your role will involve coordinating service delivery, developing care plans, and ensuring the highest quality of support is provided.

Key Responsibilities:

  • Coordinate service delivery to ensure high-quality support is provided to adults with learning disabilities and autism.
  • Develop and implement individualized care plans that meet the unique needs of each service user.
  • Work collaboratively with the multidisciplinary team to ensure seamless service delivery.
