Job Summary: We are seeking a highly motivated and experienced Care Coordination Specialist to join our team in Totnes. This is a part-time role, requiring 25 hours of work per week.

The successful candidate will be responsible for coordinating care and support services, working closely with colleagues to ensure effective communication and collaboration. This will involve developing and implementing support plans that promote independence and community engagement, as well as conducting regular reviews to ensure that all necessary adjustments are made. Additionally, you will be responsible for maintaining accurate and up-to-date records using our digital care and support app, Nourish.

To be successful in this role, you will require a full UK driving license, extensive social care experience, NVQ Level 3 in Health & Social Care, and flexibility to work evening, weekend, and holiday shifts.

We offer a competitive salary of £24,000 - £30,000 per annum, depending on experience, as well as 20 days annual leave, pension contributions, health, physiotherapy, and counselling services, training opportunities, travel-to-work scheme, and 3,500 retailer discounts.
