Rehabilitation Support Worker

We are seeking a highly motivated and compassionate Rehabilitation Support Worker to join our team at East Coast Community Healthcare CIC. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care and support to patients in our inpatient rehabilitation unit.
Key Responsibilities- Contribute to the delivery of person-centered care, promoting independence and well-being for patients.
- Work collaboratively with nursing and therapy teams to facilitate rehabilitation and support patients with activities of daily living.
- Provide physical support and assistance with personal care, including management of dietary and fluid intake, toileting programs, and continence care.
- Undertake and record observations, such as pulse, respiration rate, and blood pressure, and report any changes to healthcare professionals.
- Participate in one-to-one meetings and clinical supervision to ensure high-quality care and support.
- Contribute to the learning environment and participate in clinical audit to improve patient outcomes.
- Work as part of the multidisciplinary team to deliver delegated proactive care, including reablement and therapy exercises.
- Support therapy staff with access and discharge home visits and deliver various activities to aid functional ability.
We are looking for an individual who:
- Has previous relevant experience of working in a health or social care environment.
- Has good levels of numeracy and literacy.
- Is a team player with excellent communication and interpersonal skills.
- Is able to work independently and as part of a team, with a flexible and adaptable approach.
- Is willing to work across different sites and travel to alternative sites and across the community as required.
