About the Role

We are seeking a highly motivated and compassionate Rehabilitation Support Worker to join our established inpatient team. As a key member of our multidisciplinary team, you will play a vital role in supporting patients with activities of daily living, promoting recovery and well-being, and empowering patients to self-manage and maximise their independence.

Key Responsibilities
  • Support nursing and therapy teams to facilitate rehabilitation in an inpatient environment
  • Assist patients with activities of daily living, including personal care, dietary and fluid intake, and continence management
  • Work alongside nursing and therapy teams to deliver care that supports patients to gain as much independence as possible
  • Support therapy staff with access/discharge home visits and deliver various activities to aid functional ability
  • Work in partnership with senior members of the team to ensure the care provided is of high quality
  • Enable individuals and their families to express their feelings and preferences and take appropriate action
  • Work as part of the multidisciplinary team to deliver supportive care to patients approaching end of life
Requirements
  • Previous relevant experience of working in health or social care environment
  • Awareness of the needs of older people
  • Good interpersonal skills, good written and verbal communication skills, and ability to listen
  • Ability to accept direction and work as part of a team, but also ability to work independently without direct supervision
  • Flexibility, reliability, and ability to work all shift patterns (days/nights/weekends and bank holidays)
