About the Role

We are seeking a highly motivated and compassionate Rehabilitation Support Worker to join our established inpatient team. As a key member of our multidisciplinary team, you will play a vital role in supporting patients with activities of daily living, promoting recovery and well-being, and empowering patients to self-manage and maximize their independence.

Key Responsibilities
  • Provide hands-on care, including assisting with all aspects of personal care, such as toileting, continence management, and dietary and fluid intake.
  • Work alongside nursing and therapy teams to deliver care that supports patients to gain as much independence as possible.
  • Support end-of-life patients and their families, providing holistic symptom management and care.
  • Participate in one-to-one meetings and clinical supervision to ensure high-quality care and support.
  • Contribute to the clinical governance agenda through participation in clinical audit and quality improvement initiatives.
Requirements
  • Previous experience of working in a health or social care environment, with a focus on rehabilitation and patient-centered care.
  • Good interpersonal skills, with the ability to communicate effectively with patients, families, and colleagues.
  • Ability to work independently and as part of a team, with a flexible approach to shift patterns and travel.
