Job Summary

We are seeking an enthusiastic and motivated Healthcare Assistant to join our supportive team of nurses, healthcare assistants, nursing associates, and assistant practitioners in the West Norwich Neighbourhood Community Nursing Team. This team covers the west side of Norwich, including Costessey, Bowthorpe, Earlham, and Taverham.

Main Duties

The successful candidate will be responsible for providing high-quality care to patients in their homes, residential homes, and supported living complexes. They will work under the supervision of a qualified practitioner, following a prescribed treatment/care plan to prevent admission to hospital. The post holder will need to plan and prioritize their own visits to delegated patients, ensure patient-held records are completed for each visit, and input data into SystmOne on a daily basis.

The Healthcare Assistant will also be responsible for making basic judgments on patients' responses to prescribed treatment/care plans, reporting findings to a qualified practitioner, and liaising with other healthcare professionals as required. They will communicate with patients/carers by exchanging factual information, reassurance, tact, and empathy, and establish and maintain appropriate working relationships with colleagues, patients, carers, and other healthcare professionals.

The post holder will need to demonstrate dexterity and coordination when undertaking treatment of patients where accuracy is important, such as taking blood glucose results, wound care, and administration of medication. They will be responsible for ensuring equipment is used safely, following relevant training, and that it is maintained appropriately. The Healthcare Assistant will also need to demonstrate an awareness and understanding of consent, gain consent as appropriate following organizational policies, and share responsibility for indirect patient contact tasks, such as answering telephones, arranging appointments, processing referrals, and inputting activity data.

The successful candidate will work predominately independently on specified tasks, with regular clinical support and supervision by a qualified practitioner. They will be aware of their limitations of scope of practice and competency, and when to involve a more senior practitioner, both in daily practice and in emergency situations. The post holder will also need to acknowledge and recognize people's expressed beliefs, preferences, and choices, and demonstrate an awareness of clinical governance and risk management, and apply to the work situation.

Person Specification

The ideal candidate will have a good general education, GCSE Grade 4/C or above in English and Maths or equivalent, and experience of working within a healthcare setting. They will be able to communicate effectively using the English language in both verbal and written form, have good interpersonal skills, and be highly motivated. The post holder will also need to demonstrate empathy and sensitivity, tact and diplomacy, and be able to use their own initiative.

The successful candidate will be required to undergo a Disclosure and Barring Service check and hold a full and valid driving license and have access to a vehicle. They will also need to be passed fit to perform the full duties of the post through occupational health with any reasonable aids and adaptations if necessary.
