About the Role

University Hospitals of North Midlands NHS Trust is seeking a highly skilled and motivated Clinical Pharmacist to join our team in the Acute Medicine department.

The successful candidate will be responsible for providing clinical pharmacy services to patients and staff, working collaboratively with the multidisciplinary team to optimize patient care and outcomes.

Key Responsibilities

  • Provide clinical pharmacy services to patients and staff, including medication management and optimization
  • Collaborate with the multidisciplinary team to develop and implement patient care plans
  • Conduct medication reviews and provide recommendations for optimization
  • Develop and implement educational programs for staff and patients
  • Participate in quality improvement initiatives and audits

Person Specification

Essential Qualifications

  • Masters degree in Pharmacy (MPharm)
  • Member of the General Pharmaceutical Council (GPhC)
  • Has successfully completed a minimum of one year of a Postgraduate Diploma in Clinical Pharmacy and studying/prepared to study for Postgraduate Clinical Pharmacy Diploma / MSc or equivalent Qualification or evidence of equivalent experience or training
