Clinical Pharmacist Specialist

We are seeking a highly skilled Clinical Pharmacist Specialist to join our team at The Pharmacist Network. This is an exciting opportunity for clinically focused pharmacists who wish to expand their experience in a patient-facing role within a General Practice across the South Durham area.
Job Responsibilities- Medicine Reviews:
- Conduct telephone, face-to-face, virtual and domiciliary clinical medication reviews to proactively manage patients with complex polypharmacy.
- Review ongoing medication needs, ensuring monitoring requirements are met and opportunities to support patients achieve better health outcomes.
- Reduce inappropriate poly-pharmacy and wasteful prescribing, making appropriate recommendations.
- Repeat Prescribing:
- Advise and implement a practice repeat prescribing policy.
- Manage the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates, flagging those needing a review.
- Ensure patients have appropriate monitoring tests in place when required.
- Medicine Information:
- Provide answers to relevant medicine-related enquiries from GPs, other practice staff, healthcare teams, and patients with queries about medicines.
- Offer follow-up for patients to monitor the effect of any changes.
- Medicine Management and Risk Stratification:
- Develop and implement robust drug monitoring systems for practices, streamlining them where possible.
- Improve monitoring of high-risk medications such as anticoagulants, anticonvulsants, and DMARDs.
- Monitor practice prescribing against local guidelines and formulary recommendations.
- Telephone Medicines Support:
- Provide telephone support for patients with questions, queries, and concerns about their medicines.
- Management of Common/Minor/Self-Limiting Ailments:
- Manage patients with common/minor/self-limiting ailments while working within a scope of practice and limits of competence.
- Signpost to community pharmacy and refer to GPs or other healthcare professionals as needed.
- Unplanned Hospital Admissions:
- Review medicines commonly associated with unplanned hospital admissions and readmissions through audit and individual patient reviews.
- Implement changes to reduce prescribing of these medicines in high-risk patient groups.
- Medicines on Discharge:
- Reconcile medicines following discharge from hospitals, intermediate care, and into care homes, identifying and rectifying unexplained changes.
- Work with patients and community pharmacists to ensure continuity of medicines supply to high-risk groups.
- Signposting:
- Ensure patients are referred to the appropriate healthcare professional for the appropriate level of care within an appropriate timeframe for pathology results, common/minor ailments, acute conditions, long-term condition reviews, etc.
- Medicines Safety and Quality Improvement:
- Implement changes resulting from MHRA alerts, product withdrawals, and local/national guidance.
- Undertake clinical audits of prescribing, providing feedback and implementing changes with the practice team.
- Implementation of Local and National Guidelines:
- Monitor practice prescribing against the local health economy's RAG list, recommending medicines that should be prescribed by hospital doctors (red drugs) or subject to shared care (amber drugs).
- Assist practices in maintaining a practice formulary hosted on the practice computer system.
- Audit compliance against NICE technology assessment guidance.
- Patient-Facing Clinical Medication Review:
- Undertake clinical medication reviews with patients, producing recommendations for senior clinical pharmacists, nurses, and/or GPs on prescribing and monitoring.
- Review ongoing medication needs for patients with long-term conditions, including monitoring requirements and opportunities to support patients with their medicines taking.
- See patients with single or multiple medical problems where medicine optimisation is required, offering signposting to community pharmacy and referring to GPs or other healthcare professionals as needed.
