Are you passionate about delivering high-quality care to patients with complex neurological conditions? As a specialist Adult Community Neurological Physiotherapist at Hertfordshire Community NHS Trust, you will have the opportunity to make a real difference in the lives of our patients.

You will work as part of a multidisciplinary team, providing specialist assessment and treatment sessions to a caseload of approximately 5-6 patients. You will have access to a range of specialist equipment, including a partial body weight support system, gait analysis treadmill, and FES bike.

Main duties of the job
  • To provide high-quality assessment and treatment packages for patients with complex neurological conditions.
  • To provide expert advice, guidance, and information to health and social care professionals, carers, and relatives.
  • To contribute to the training, development, and supervision of junior staff, rehabilitation assistants, and students.
  • To take a lead in the rehabilitation and discharge planning of patients as part of the 'lead therapist' role, which involves chairing patient/family meetings and making onward referrals.

Job responsibilities

To undertake thorough assessments of patients, using advanced clinical reasoning skills and manual assessment techniques to provide accurate diagnoses.

To be responsible for your own caseload and work independently, with supervision taking the form of regular formal training and clinical reasoning sessions, peer review, case conferences, and reflective practice.

To develop and implement advanced physiotherapy assessment and treatment plans for patients with stroke and other complex neurological conditions.

To assess and manage clinical risk within your patient caseload.

To represent the physiotherapy service and/or individual patients at multi-disciplinary team meetings.

To ensure the delivery of a coordinated multidisciplinary service, integrating physiotherapy treatment into the treatment programme.

To be responsible for planning and organising your own caseload to meet service and patient priorities.
