Livewell Southwest, a leading healthcare provider, is seeking an experienced Adoption Caseworker to join their team in Plymouth, South Hams, and West Devon.
About the RoleThe ideal candidate will have a strong background in social work, with excellent communication and interpersonal skills. The role involves managing a caseload of vulnerable individuals, conducting holistic assessments, and developing care plans to promote independence and well-being.
Key Responsibilities:
- Manage a caseload of clients with complex needs, including mental health, learning disabilities, and physical disabilities.
- Conduct comprehensive assessments to identify strengths and needs, and develop personalized care plans.
- Work closely with multidisciplinary teams, including physiotherapists, occupational therapists, and nurses, to provide seamless care services.
- Communicate effectively with clients, carers, and other stakeholders to ensure informed decision-making and collaboration.
- Stay up-to-date with relevant laws, policies, and guidelines, including the Care Act 2014, Mental Capacity Act 2005, and Human Rights Act 1998.
To be successful in this role, you will need:
- A degree in Social Work or a related field.
- A valid Social Work England registration.
- Proven experience working in healthcare or social care, with a focus on vulnerable populations.
- Excellent communication, interpersonal, and problem-solving skills.
