Job Summary:

This role will involve managing a caseload of clients who are socially disconnected and isolated, providing them with personalised assessment, signposting, information, and support to help them access local groups and services independently. You will also facilitate the development and delivery of personal action/activity plans, working closely with clients to maintain motivation and assess progress against their individualised plan.

  • This role requires excellent communication and interpersonal skills, as well as the ability to work effectively in a hybrid environment.
  • You will have experience of working with vulnerable adults, preferably in a community or wellbeing setting.
  • Our ideal candidate will be able to provide clear guidance on accessing local services, ensuring that clients receive the right support at the right time.
Key Responsibilities:
  1. Manage a caseload of clients who are socially disconnected and isolated, providing them with personalised assessment, signposting, information, and support.
  2. Facilitate the development and delivery of personal action/activity plans, working closely with clients to maintain motivation and assess progress.
  3. Provide clear guidance on accessing local services, ensuring that clients receive the right support at the right time.
  4. Work effectively in a hybrid environment, collaborating with colleagues to achieve shared goals.
  5. Maintain accurate records of client interactions, outcomes, and progress, contributing to performance monitoring and evaluation.

We offer a competitive salary of £23,000 - £25,000 per annum, depending on experience, as well as opportunities for professional development and training.
