Job Summary

As a key member of the Ashford and St. Peter's Hospitals NHS Foundation Trust team, you will play a vital role in managing and leading the outreach anticoagulation clinics at Woking Community Diagnostic Hub. Your expertise will be instrumental in implementing treatment plans for patients, collaborating with healthcare professionals, and contributing to the development of the anticoagulant service.

Main Responsibilities
  • Manage and run the anticoagulant outreach clinics at Woking Community Diagnostic Hub, ensuring seamless coordination with GP, SPA, and District Nurse Teams.
  • Develop and implement treatment plans for patients, utilizing evidence-based best practices and adhering to Trust protocols.
  • Collaborate with multidisciplinary teams to enhance patient care and service delivery.
  • Contribute to the development of the anticoagulant service, proposing innovative solutions to meet the needs of patients and the hospital.
  • Maintain accurate and up-to-date computerized records using DAWN and Cerner software.

Person Specification

We are seeking a highly skilled and experienced individual with a strong background in anticoagulation and a passion for delivering exceptional patient care. The ideal candidate will possess:

  • 1st Level Registered Nurse or MPharm degree with integrated foundation training.
  • Completed competencies for Anticoagulation Clinical Nurse Specialist assessed by Consultant Hematologist.
  • Experience within Anticoagulation at Specialist level.
  • Experience of working within a multidisciplinary team.
