Job Summary

We are seeking a dedicated and compassionate individual to work with our team in the North and South Derbyshire Dales, providing high-quality support to older adults with functional mental health conditions.

Main Responsibilities

The successful candidate will be responsible for working with older adults in a community setting, providing person-centred support to enable them to live their best life. This will involve:

  • Conducting comprehensive psychosocial assessments and risk assessments to identify the needs of older adults
  • Developing and implementing individualised care plans to support older adults in achieving their goals
  • Working collaboratively with colleagues and community partners to ensure seamless support and effective service delivery
  • Providing education and training to older adults and their families on mental health and wellbeing
