About the Role

We are seeking a highly skilled and compassionate Older Adult Practitioner to join our team at Derbyshire Healthcare NHS Foundation Trust. As a key member of our community mental health team, you will play a vital role in supporting older adults with functional mental health conditions in a community setting.

Main Responsibilities
  • Provide comprehensive person-centred psychosocial assessments and risk assessments to identify the mental health needs of older adults.
  • Develop and implement individualized care plans to support older adults in achieving their mental health goals.
  • Build therapeutic relationships with older adults and their families to empower them to take control of their mental health.
  • Collaborate with colleagues and community partners to ensure seamless support and coordination of care.
Key Requirements
  • Essential Qualifications: Diploma in Nursing or Degree in Mental Health Nursing or Degree in Occupational Therapy.
  • Desirable Qualifications: Assessor qualification or further training relevant to the role.
  • Essential Experience: Post-qualification experience in the relevant field, experience working with older people with functional mental health diagnoses, and some experience of community working.
  • Desirable Experience: Experience of linking with partner agencies.
