About the Role

We are seeking an experienced and compassionate Older Adult Practitioner to join our team at Derbyshire Healthcare NHS Foundation Trust. As a key member of our community mental health team, you will play a vital role in supporting older adults with functional mental health conditions in a community setting.

Key Responsibilities
  • Conduct comprehensive person-centred psychosocial assessments and risk assessments to identify mental health needs.
  • Develop and implement person-centred care plans to empower individuals to take control of their health.
  • Build therapeutic relationships and work collaboratively with colleagues, partners, and community members to develop services that meet the mental health needs of the community.
  • Support and motivate individuals to improve their mental health by helping them develop self-care skills, build resilience, and better utilise community resources.
Requirements
  • Relevant qualification in nursing or occupational therapy (Diploma in Nursing/Degree in Mental Health Nursing or Degree in Occupational Therapy).
  • Post-qualification experience in the relevant field, including experience working with older adults with functional mental health diagnoses.
  • Experience of community working and linking with partner agencies.
