Job Summary

This is an exciting opportunity to join Derbyshire Healthcare NHS Foundation Trust as a skilled professional in the field of older adult care. We are seeking a dedicated and compassionate individual to work with our team in the North and South Derbyshire Dales, providing high-quality support to older adults with functional mental health conditions.

Main Responsibilities

The successful candidate will be responsible for working with colleagues across the local community to ensure that we provide the right service at the right time by the right person. This will involve supporting introductions into the Living Well Team, identifying the needs of older adults with mental health needs, and designing and implementing person-centred support to enable people to live their best life.

The role will also involve working collaboratively with colleagues, key partners, and those within the local community to develop services that meet the mental health needs of the community. This will include undertaking comprehensive person-centred psychosocial assessments, risk assessments, and formulation of presenting mental health needs.

About the Role

This is a unique opportunity to work in a friendly and supportive team, making a positive difference in the lives of older adults. The role will involve building therapeutic relationships, developing a plan of care with individuals, and assisting them through difficult times by empowering and enabling them to take control over their health.

The post holder will be clinically responsible for supporting the delivery of a flexible, responsive local service, taking forward an Integrated Wellbeing Pathway in the designated locality. This will involve working collaboratively with colleagues and key partners to develop services that meet the mental health needs of the community.

Person Specification

We are looking for an individual with the following qualifications and experience:

  • Relevant qualification for the post (Diploma in Nursing/Degree in Mental Health Nursing) or Degree in Occupational Therapy
  • Post-qualification experience in the relevant field
  • Experience of working with older people with functional mental health diagnoses
  • Experience of community working
