Job Title: Older Adult Practitioner - Living Well Specialist

We are seeking an experienced and compassionate Older Adult Practitioner to join our team at Derbyshire Healthcare NHS Foundation Trust. As a key member of our Living Well Team, you will play a vital role in supporting older adults with functional mental health conditions in our community.

About the Role:

As an Older Adult Practitioner, you will be responsible for providing person-centred care, working collaboratively with colleagues and community partners to develop services that meet the mental health needs of our community.

Key Responsibilities:
  • Provide comprehensive person-centred psychosocial assessments and risk assessments for older adults with functional mental health conditions.
  • Develop and implement person-centred care plans, working collaboratively with patients, families, and community partners.
  • Build therapeutic relationships with patients, empowering them to take control of their health and well-being.
  • Support and motivate individuals to improve their mental health, helping them develop self-care skills and build resilience.
  • Work collaboratively with community partners, including statutory organisations, Voluntary, Community and Social Enterprise (VCSE) organisations, and local communities.
Person Specification:

We are looking for an experienced and qualified Older Adult Practitioner who possesses the following skills and qualities:

  • Relevant qualification in nursing or occupational therapy.
  • Post-qualification experience in working with older adults with functional mental health conditions.
  • Experience of community working and working with partner agencies.
  • Ability to work collaboratively with colleagues and community partners.
  • Strong communication and interpersonal skills.
