We are seeking a qualified and compassionate Registered Nurse to join our team as a Palliative Care Coordinator. As a Palliative Care Coordinator, you will work closely with our multidisciplinary team to provide comprehensive home nursing services tailored to meet the unique needs of individuals, families, and groups facing end-of-life circumstances.
You will be responsible for assessing, monitoring, and making care planning/care coordination decisions, as well as ensuring the quality of care of patients by identifying and acting on factors which affect practice autonomy and delivery of quality care.
