We are seeking a highly motivated Care Coordinator to support individuals with disabilities in their daily activities and personal development.
Key Responsibilities:- Provide individualized support in daily living activities and encourage independence.
- Develop and implement personalized care plans, monitoring progress and making adjustments as needed.
- Facilitate community and social outings for service users, promoting engagement and connection.
- A high school diploma or equivalent is required; certification in healthcare or social work is an asset.
- Previous experience working with individuals with disabilities is advantageous.
- Strong communication and interpersonal skills, with the ability to build rapport with clients and colleagues.
