Job Summary
We are seeking a highly skilled and experienced Field Care Supervisor to join our team. As a Field Care Supervisor, you will play a crucial role in ensuring we deliver exceptional care support services to our clients.
Key Responsibilities
- Support and Development of Care Workers
- Provide guidance, instruction, and support to our care workers to ensure they meet the highest standards of care.
- Care Service Planning and Delivery
- Conduct assessments and reviews of individual services to ensure our clients receive the best possible care.
- Collaborate with care coordinators to plan effective and realistic care worker rotas that align with the specific needs of our clients and the skills of our care workers.
- Performance Monitoring and Quality Assurance
- Perform on-site observations and evaluations of care worker practices as part of routine supervision or in response to performance concerns.
- Multi-Disciplinary Collaboration
- Participate in multi-disciplinary reviews of client needs to provide comprehensive and coordinated care.
- Training and Mentorship
- Shadow new care workers in the field to ensure their competence and confidence in delivering exceptional care.
Requirements
- Assessment and Planning Experience
- Proven experience in conducting initial risk and needs assessments for individual clients, whether in their homes or in other settings.
- Collaborative Approach
- Ability to work in partnership with clients, their families, carers, advocates, healthcare professionals, and others to ensure the seamless delivery of comprehensive, holistic care packages.
