Job Summary

We are seeking an experienced and passionate individual to join our team as a Field Care Supervisor. As a Field Care Supervisor, you will play a crucial role in ensuring we deliver exceptional care support services to our clients.

Key Responsibilities

  • Support and Development
    • Provide guidance and support to our care workers to ensure they meet the highest standards of care.
    • Conduct regular assessments and reviews of individual services to ensure our clients receive the best possible care.
  • Care Planning and Coordination
    • Collaborate with care coordinators to plan effective and realistic care worker rotas that align with the specific needs of our clients and the skills of our care workers.
  • Performance Monitoring and Evaluation
    • Perform on-site observations and evaluations of care worker practices as part of routine supervision or in response to performance concerns.
  • Multi-Disciplinary Collaboration
    • Participate in multi-disciplinary reviews of client needs to provide comprehensive and coordinated care.
  • Training and Mentorship
    • Shadow new care workers in the field to ensure their competence and confidence in delivering exceptional care.

Requirements

  • Assessment Experience
    • Proven experience in conducting initial risk and needs assessments for individual clients, whether in their homes or in other settings.
  • Collaborative Approach
    • Ability to work in partnership with clients, their families, carers, advocates, healthcare professionals, and others to ensure the seamless delivery of comprehensive, holistic care packages.
