Role: Field Care Supervisor

Are you an experienced Care Assistant seeking a rewarding opportunity to make a tangible difference? We are actively searching for passionate individuals to join our team as a Field Care Supervisor.

What You'll Do:
  • Support and Guidance: Provide practical supervision, instruction, and support to our dedicated care workers, ensuring they consistently deliver the highest quality of care.
  • Assessment and Review: Conduct comprehensive assessments and reviews of individual care services, including regular needs and risk assessments, to guarantee our service users receive optimal care.
  • Care Planning Collaboration: Work closely with Care Coordinators to develop effective and realistic care worker rotas that align perfectly with the specific needs of our service users and the skills of our care workers.
  • Performance Monitoring: Conduct on-site observations and evaluations of care worker practices as part of routine supervision or in response to performance concerns, ensuring adherence to high standards.
  • Multidisciplinary Teamwork: Actively participate in multi-disciplinary reviews of service users' needs to contribute to comprehensive and coordinated care plans.
  • Training and Mentorship: Guide and mentor new care workers in the field, ensuring their competence and confidence in delivering exceptional care.

While a significant portion of your time will be dedicated to community-based care, you will also handle essential administrative tasks to support our overall care operations.

What You Need:
  • Assessment Expertise: Proven experience in conducting initial risk and needs assessments for individual service users, whether in their homes or other settings such as hospitals, residential, or nursing homes.
  • Collaborative Approach: A strong ability to work collaboratively with service users, their families, carers, advocates, healthcare professionals, and others to ensure the seamless delivery of comprehensive, holistic care packages.
